INDIVIDUAL SUPPORT PLAN (ISP)
At an annual planning meeting, each participant invites persons from their support circle to gather and discuss the individual’s choices, aspirations, needs and wants. A plan is developed with goals and objectives to assist that person in reaching his/her potential. People and/or agencies are selected to assist the participant in realizing these goals and objectives. A process is established by which the progress and success of the plan will be measured and reviewed. At all times the Agency remains cognizant of the fact that this plan is designed and authorized by the participant and should be carried out in the least restrictive settings as possible.
